WHO WE ARE

The PHC Foundation is a mission driven non-profit 501(c)(3) organization developed to help meet the growing demand for health and personal care services in the tri-state.

Since opening its doors in 2005, the PHC Foundation has provided services for well over 100 clients, averaging 3 to 5 new clients each month.  

The parent company, Private Home Care, Inc., has been

providing quality health care services in our community since 1991.
